Section C
Obstetrics Section: Products of Conception
Procedures performed on the products of conception are coded to the Obstetrics section. This is a critical distinction in medical coding that ensures accurate documentation and billing for obstetric procedures.
Procedures performed on the pregnant female other than the products of conception are coded to the appropriate root operation in the Medical and Surgical section. Understanding this distinction is essential for proper code assignment.
Products of Conception
Amniocentesis procedures are coded to the products of conception body part in the Obstetrics section
Maternal Procedures
Repair of obstetric urethral laceration is coded to the urethra body part in the Medical and Surgical section

Post-Delivery and Post-Abortion Procedures
Obstetrics Section Coding
Procedures performed following a delivery or abortion for curettage of the endometrium or evacuation of retained products of conception are all coded in the Obstetrics section, to the root operation Extraction and the body part Products of Conception, Retained.
Medical and Surgical Section
Diagnostic or therapeutic dilation and curettage performed during times other than the postpartum or post-abortion period are all coded in the Medical and Surgical section, to the root operation Extraction and the body part Endometrium.
01
Identify Timing
Determine if procedure is postpartum/post-abortion or other period
02
Select Section
Choose Obstetrics or Medical and Surgical section accordingly
03
Assign Code
Apply appropriate root operation and body part values

Section D
Radiation Therapy: Brachytherapy Basics
Brachytherapy is coded to the modality Brachytherapy in the Radiation Therapy section. This specialized form of radiation therapy involves placing radioactive sources directly at the treatment site.
When a radioactive brachytherapy source is left in the body at the end of the procedure, it is coded separately to the root operation Insertion with the device value Radioactive Element. This dual coding approach captures both the radiation treatment and the implantation procedure.

Brachytherapy Coding Example
Example: Brachytherapy with implantation of a low dose rate brachytherapy source left in the body at the end of the procedure is coded to the applicable treatment site in section D, Radiation Therapy, with the modality Brachytherapy, the modality qualifier value Low Dose Rate, and the applicable isotope value and qualifier value.
The implantation of the brachytherapy source is coded separately to the device value Radioactive Element in the appropriate Insertion table of the Medical and Surgical section. The Radiation Therapy section code identifies the specific modality and isotope of the brachytherapy, and the root operation Insertion code identifies the implantation of the brachytherapy source that remains in the body at the end of the procedure.

2
Insertion Code
Medical and Surgical section: Insertion of Radioactive Element device value

Special Brachytherapy Exceptions
Cesium-131 Collagen Implant
Implantation of Cesium-131 brachytherapy seeds embedded in a collagen matrix to the treatment site after resection of brain tumor is coded to the root operation Insertion with the device value Radioactive Element, Cesium-131 Collagen Implant.
Palladium-103 Collagen Implant
Implantation of Palladium-103 brachytherapy seeds embedded in a collagen matrix to the treatment site after resection of brain tumor is coded to the root operation Insertion with the device value Radioactive Element, Palladium-103 Collagen Implant.

Temporary Brachytherapy Applicators
A separate procedure to place a temporary applicator for delivering the brachytherapy is coded to the root operation Insertion and the device value Other Device. This distinction is important for accurate procedure documentation.
1
Separate Applicator Placement
Intrauterine brachytherapy applicator placed as a separate procedure from the brachytherapy procedure is coded to Insertion of Other Device, and the brachytherapy is coded separately using the modality Brachytherapy in the Radiation Therapy section.
2
Concomitant Placement
Intrauterine brachytherapy applicator placed concomitantly with delivery of the brachytherapy dose is coded with a single code using the modality Brachytherapy in the Radiation Therapy section.

Section X
New Technology Section Guidelines
Section X codes fully represent the specific procedure described in the code title, and do not require additional codes from other sections of ICD-10-PCS. When section X contains a code title which fully describes a specific new technology procedure, and it is the only procedure performed, only the section X code is reported for the procedure.
Example: XW033CA, Introduction of zanidatamab antineoplastic into peripheral vein, percutaneous approach, new technology group 10, should be assigned to indicate that zanidatamab antineoplastic was administered via a peripheral vein. A separate code from table 3E0 in the Administration section of ICD-10-PCS is not assigned in addition to this code.
Complete Documentation
Section X codes are comprehensive and self-contained
No Additional Codes
Do not assign codes from other sections when Section X applies

Multiple New Technology Procedures
When multiple procedures are performed, New Technology section X codes are assigned following the multiple procedures guideline. This ensures comprehensive documentation of all innovative treatments provided.
TAVR with Leaflet Laceration
To identify the division of aortic valve leaflets using radiofrequency energy during a valve-in-valve transcatheter aortic valve replacement (TAVR), code X28F3WB, Division of aortic valve using leaflet laceration, radiofrequency energy, percutaneous approach, new technology group 11, is assigned along with a separate code for the TAVR procedure.
Gene Therapy Administration
To identify the administration of eladocagene exuparvovec-tneq using a temporary infusion device, code XW0Q316, Introduction of eladocagene exuparvovec into cranial cavity and brain, percutaneous approach, new technology group 6, is assigned along with a separate code to identify the insertion of the temporary infusion device.

Selection of Principal Procedure
The following instructions should be applied in the selection of principal procedure and clarification on the importance of the relation to the principal diagnosis when more than one procedure is performed.
1
Definitive Treatment for Both Diagnoses
Sequence procedure performed for definitive treatment most related to principal diagnosis as principal procedure
2
Definitive and Diagnostic Procedures
Sequence procedure performed for definitive treatment most related to principal diagnosis as principal procedure
3
Diagnostic for Principal, Definitive for Secondary
Sequence diagnostic procedure as principal procedure, since the procedure most related to the principal diagnosis takes precedence
4
No Procedures Related to Principal Diagnosis
Sequence procedure performed for definitive treatment of secondary diagnosis as principal procedure
