You'll learn how to enter ICD-10-CM diagnosis codes accurately into the 3M encoder, navigate code suggestions, apply clinical documentation guidelines, and ensure that each diagnosis is supported by the medical record. Begin with the discharge summary and history & physical to understand the patient's clinical picture.

Emphasis will be placed on selecting the appropriate root operation, body part, approach, and device characters — the building blocks of every valid ICD-10-PCS code.
Apply the complete inpatient review methodology to identify all billable and reportable procedures documented in the chart by the surgeon, physician, or operative team.
Navigate the 3M PCS tables to build procedure codes character by character — mastering root operations, body parts, approaches, devices, and qualifiers.
Learn best practices for ensuring your procedure codes are fully supported by operative reports and documentation — critical for audit-proof coding.

Confirm that the DRG generated by the encoder accurately reflects the coded diagnoses and procedures documented in the medical record.
