Day 1 — Monday, May 4
Foundations of Inpatient Medical Records
The workshop kicks off by building a rock-solid foundation in understanding how inpatient medical records are structured, reviewed, and used in the coding process. On Day 1, you will learn how to systematically approach and review a complete inpatient medical record — an essential skill that separates accurate coders from the rest.
You'll explore the key elements that make up an inpatient chart, including history and physical notes, operative reports, discharge summaries, physician orders, and diagnostic test results. Understanding the format and layout of these documents is critical before you can identify and assign the correct diagnosis and procedure codes.
How to Review Inpatient Medical Records
Learn a systematic, structured approach to reviewing complete patient charts with confidence and accuracy.
Medical Record Elements and Formats
Understand the components of an inpatient record — from admission notes to discharge summaries — and how each informs your coding decisions.
Day 2 — Tuesday, May 5
Tools for IP DRG Projects & 3M Introduction
Day 2 dives into the technology and tools that power inpatient coding in real-world DRG (Diagnosis-Related Group) projects. Understanding which tools are used and how to navigate them is one of the most in-demand skills employers look for in inpatient coders today.
A key highlight of this session is a comprehensive introduction to the 3M Coding and Reimbursement System — the industry-leading encoder software widely used in hospitals and healthcare facilities across the United States. You'll get a guided navigation overview and learn the interface from the ground up, so you feel confident using it from day one on the job.
Tools Used in IP DRG Projects
Discover the essential software platforms, encoders, and reference tools that professional inpatient coders rely on every day in DRG-based projects.
3M Tool — Introduction & Navigation
Get your first hands-on look at the 3M Coding and Reimbursement System. Learn to navigate its interface, access code sets, and understand how it supports DRG assignment and validation.
  • 3M platform overview and login navigation
  • Accessing ICD-10-CM/PCS code sets
  • Understanding the encoder workflow
Day 3 — Wednesday, May 6
Step-by-Step Record Review & Adding Diagnoses
By Day 3, you move from concepts to hands-on application. This session walks you through the complete, step-by-step inpatient record review process — the exact workflow a professional coder follows from the moment they open a chart to when they finalize their code selections.
The second half of the session focuses on one of the most critical practical skills: adding diagnoses using the 3M software. You'll learn how to enter ICD-10-CM diagnosis codes accurately into the 3M encoder, navigate code suggestions, apply clinical documentation guidelines, and ensure that each diagnosis is supported by the medical record. This is where classroom knowledge meets real-world application.
01
Open and Assess the Chart
Begin with the discharge summary and history & physical to understand the patient's clinical picture.
02
Review Supporting Documentation
Cross-reference operative notes, lab results, pathology reports, and physician orders for complete clinical evidence.
03
Enter Diagnoses in 3M
Use the 3M encoder to search, validate, and assign ICD-10-CM diagnosis codes with proper specificity and documentation support.
Day 4 — Thursday, May 7
Adding Procedures with the 3M Software
Day 4 builds directly on the previous session, extending the record review workflow to include ICD-10-PCS procedure coding — one of the most complex and technical components of inpatient coding. The ICD-10-PCS system uses a seven-character code structure, and navigating it accurately requires both clinical understanding and software proficiency.
You'll continue working through the step-by-step inpatient record review process, this time with a focus on identifying documented procedures, understanding the PCS code structure, and entering procedure codes correctly into the 3M encoder. Emphasis will be placed on selecting the appropriate root operation, body part, approach, and device characters — the building blocks of every valid ICD-10-PCS code.
Step-by-Step Record Review (Procedures)
Apply the complete inpatient review methodology to identify all billable and reportable procedures documented in the chart by the surgeon, physician, or operative team.
Adding Procedures in 3M
Navigate the 3M PCS tables to build procedure codes character by character — mastering root operations, body parts, approaches, devices, and qualifiers.
Accuracy and Compliance
Learn best practices for ensuring your procedure codes are fully supported by operative reports and documentation — critical for audit-proof coding.
Day 5 — Friday, May 8
Principal Diagnosis, DRG Validation & Final Coding
The final day of the workshop brings everything together in a comprehensive, high-impact session covering the most strategically important elements of inpatient coding: identifying the principal diagnosis, validating the DRG, mastering sequencing priority, and finalizing your coding submission.
Identifying the Principal Diagnosis is arguably the single most impactful coding decision you'll make on any inpatient case. It directly determines the DRG assignment and, therefore, the reimbursement the hospital receives. You'll learn the UHDDS (Uniform Hospital Discharge Data Set) definition and how to apply it with accuracy under various clinical scenarios.
Principal Diagnosis
Apply UHDDS guidelines to correctly identify the condition established after study to be chiefly responsible for the admission.
DRG Validation
Confirm that the DRG generated by the encoder accurately reflects the coded diagnoses and procedures documented in the medical record.
Sequencing Priority
Master the rules and guidelines that govern how diagnoses and procedures are sequenced to optimize accuracy, compliance, and reimbursement integrity.
Final Coding Updates
Review, finalize, and submit your complete code set with confidence — applying all the skills developed across the five-day workshop.
